In heavy equipment, it has been observed that
operators are covering the equipment cabin
glasses with the cardboards to protect themselves
from sunlight (for the purpose of shade)
unknowingly creating blind spots while operation
of heavy equipment and in turn can lead to
accidents.

 KEY FINDINGS:
• Lack of competency, training and awareness.
• Lack of adequate supervision and monitoring
• Lack of Daily routine inspections to ensure safe
practices
 LESSON LEARNED AND KEY MESSAGES:
• Enhance training program for operators
regarding safe operation of equipment.
• Enhance supervision by management for the all
equipment and operators to ensure proper
working conditions.
• It is solely the responsibility of the supervisor to
ensure adequate provisions are in place and are
continuously monitored in order to arrange
additional tinting of windows or shade stickers.
• Report unsafe acts and unsafe conditions to
avoid incidents and accidents.
