This is to bring to your kind consideration that due to a small accident that I met in the collage
stairs on 15th September 2019, I had an ankle sprain and a small injury in ligament, due to which
I had a plaster on my Right Leg and had to be in bed rest for 2 weeks as per Doctor’s
prescription. Because of this I was not able to attend any class in the last week from 16 th
September to 20th September 2019, also could not submit any assignment.

I therefore urge you to accept my application and kindly allow me a leave of 1 week from 16 th
September to 20th September. Also an extension of 1 week for the assignments. Along with this I
would request to shift our class to ground floor, so that it would be easier for me to attend the
class, because as per doctor’s prescription using stairs is not safe for 1 more week.

For this I shall be highly obliged to you and also assure you the timely submission of my
assignments as per extension.

verification.)
