National Headquarters/National Support Units Training Center, after having been duly sworn
in accordance with law, hereby depose and state:

That, I fully understood all the provisions set forth in the NPTI Training Guide Revised
2018 and will abide with the rules and regulations stated therein;

That, I will never engage in any form of violence and/or harassment with other students
or trainees and aware of the consequences and possible deferment from the training aside
from criminal and administrative liabilities may impose;

That, I hereby undertake to comply with the BMI program of the PNP and once found
obese and overweight category after series of BMI measurements & conduct of weight
management program it will affect and delay my graduation until I reach the normal or
acceptable BMI;
