ONLINE SELF-MONITORING
REPORT SYSTEM
(OPERATIONAL MANUAL)
BACKGROUND OF SELF MONITORING
REPORT

▪ Submission of Self-Monitoring Report (SMR) is required under BR


No. 33, Series of 1996, or the Rules and Regulations
Implementing the Environmental User Fee in the Laguna de Bay
Region;

▪ Stated under Article III, Section 9 of the same BR, submission of


SMR by permit holder or discharger shall be on a quarterly basis
and should be signed by the Chief Executive Officer and the
Pollution Control Officer;

▪ The report shall contain specifically the quality and quantity of


wastes discharged daily or periodically, as the case may be, the
characterization and laboratory analyses conducted preferably by
a duly licensed and accredited laboratory of the Authority, and
such other material information the Authority may require from the
discharger.

PROCEDURES IN USING THE ONLINE SELF-
MONITORING REPORT SYSTEM
(Step by Step)

Step 1. Go to your internet browser

Step 2. Search (http://onlineservices.llda.gov.ph/OSMR) on the address bar

Step 3. For new user, click the “Not Yet Registered” button and then
proceed to the user’s registration process.

Step 4. Simply fill-up those needed information. Make sure that use the
registered Establishment ID indicated to your Discharge Permit.

➢ Make sure to fill-up all those with *

➢ Remember the email used as well as


the password entered.

➢ Click the “Register” button and then


verification email was sent on the
registered email that you used on the
system.

Step 5. Verification email was already sent on the email used during
registration.

Step 6. Check your inbox, and you’ll have this kind of email.

Click the “Verify Email Address” button, and then you will be redirected to the
next process.

Again, fill-up those needed information. Make sure that it is correct and
updated.

For registration
process compliance,
you also need to
provide some of your
company’s general
information.

Step 7. After filling-up those needed information for registration process,


you can now create SMR (Reports) by just clicking the “+” button on the top
right of the screen.

Step 8.

Input the covered year indicated


to your Discharge Permit (DP),
Item No. 1
Input quarter

Click “Ok” button

Step 9. Fill-up now the Wastewater Treatment Plant (WTP) Record.

Click this button to


input the data
needed to comply
the WTP Record

Step 10. For the first part, just click the check box which indicates the day of
the Month.

Tick the Checkbox to
Group Selection and
then click button
“Group Selection”

Step 11. Input also the Effluent Flow Rate after clicking the group selection
button.

Be sure to input
the Effluent Flow
Rate and click “Ok”
button.

If all data are exact


and correct, then
click the “Save
Changes” button.

Step 13. The first month’s data was already saved. Proceed now to the
following more months of your WTP Record.

After completing all the
data for 3 months WTP
Record, click the “Save and
Next” button to proceed to
the next page of the
system.

Make sure that “SMR –
Waste Treatment Plant
Record is successfully
saved” will appear after
clicking the “Save and
Next” button.

Same procedure,
simply click the “+”
button to add data on
the next report for SMR

Step 14. Next, will be the Monthly Report of Wastewater Characteristics


(Conventional Pollutants)

Clicking the “+” button,


will lead to the next
data needed. The
Parameter Name &
Unit.

And then click the


“Save” button.

Same procedure on the


first one.

After, input also the Name


of the Laboratory, Address
and Phone Number.

Click “Save and Next”


button to proceed to the
next report.

Step 15. Next report will be, Monthly Report of Wastewater Characteristics
(Other Pollutants). The same procedure on the previous report for
conventional pollutants they only differ on the data that will be
provided.

After all data are filled-up,


click the “Save and Next”
button.

Step 16. Next report, will be the Sources of Wastewater during this Quarter.
All procedures will still be the same. They only vary on the data that
will encode.

After filling-up those
needed information, you
can now click the “Save
and Next” button.

Step 17. After filling-up the last report needed for SMR, you can now finally
submit your report and it will be automatically listed on the
“Submitted Reports” as shown on Step 7.

Before you submit your


SMR, click this “Choose
File” if you have any to
attach, like ROLA.

Step 18. You can now select the file that you will attached together with the
SMR.

After you attached a


certain file, you can
now click the
“Submit” button.

picture, your SMR is
now listed on the
Submitted Reports.
