 CENTENARIAN CASH GIFT

INSTRUCTIONS:

A. Eligibility - Be in good standing. A member in good standing is one who complies with the
duties and responsibilities prescribed by the VFP, to include payment of membership dues, and
does not perform acts derogatory and inimical to the interest of the VFP.

B. Scope and Coverage:

1. Grant of Centennial Cash Gift of Fifty Thousand Pesos (P50,000) shall be one time basis only.

2. It shall apply to a living veteran who reaches the age of one hundred (100) years old and above
and a member in good standing of the VFP.

C. Documentary requirements:

1. Duly accomplished application form to avail of the Centenarian cash gift duly endorsed by the
Post/Charter/Affiliate President.

2. Original copy of birth certificate issued by Philippine Statistic Authority (PSA) or Local Registrar
(LCR) with registry number or if authenticated, it must be authenticated original copy with an
official dry seal of the Local Civil Registrar (LCR) or original copy of any government document
duly authenticated with an official dry seal by the Local Civil Registrar (LCR) .

3. Photocopy of PVAO Pensioner ID card and VFP membership ID card.

4. Three (3) specimen signatures of the applicant member

D. Mode of Payment:

The centenarian cash gift in the amount of Fifty Thousand Pesos Only (PHP50,000) in check will
be paid directly to qualified VFP member in good standing who apply for this benefit upon
submission of accomplished application form and original documentary requirements to National
Headquarters, Veterans Federation of the Philippines (VFP).
