WAIVER AGREEMENT

I, __________________________________, the undersigned parent/legal guardian of the

child/youth named ____________________________________, who was born on
_____________________________ and resides in
____________________________________________________.

For any situation, I assure that I will be available for the phone call at ______________________. As
a parent/legal guardian, I affirm that I allow my child to participate in attending a Wedding
celebration dated __________________________________ in this time of Pandemic at THE BELLA
PLAZA.

I further acknowledge that THE BELLA PLAZA has put in place preventive measure to reduce the
spread of the Corona Virus.

I further acknowledge that THE BELLA PLAZA cannot guarantee that I and my child will not become
infected with the Covid19.I understand that the risk of becoming exposed to and/or infected by the
Covid19 may result from the actions,omissions, or negligence of myself and others, including, but not
limited to, staff, and other visitors and their families.

I voluntarily seek service provided by THE BELLA PLAZA and acknowledge that I am increasing my risk
together with my child to expose to the Corona Virus. I acknowledge that we must comply will all set
procedures to reduce the spread while attending the said event.

I hereby release and agree to hold THE BELLA PLAZA harmless from, and waive on behalf of myself,
my heirs, and any personal representatives any and all causes of actions, claims, demands,
damages,costs, expenses and compensation for damages or loss to myself and/or property that me
be caused by any act, or failure to act of the Events place, or that may otherwise arise in any way in
connection with any services received from THE BELLA PLAZA. I understand that this released
discharges THE BELLA PLAZA from any liability or claim that I, my heirs, or any personal
representatives may have against the Events place with respect to any bodily injury, illness, death
medical treatment, or property damage that may arise from, or in connection to, any services
received from THE BELLA PLAZA. This liability waiver and released extends to the Events Place
together with all owners, partners and employees.
