Bonnin (Director) oe Pall Pal) \ aiegemnity / In accordance with instructions received from WEST OF ENGLAND P&l CLUB, we attended on board the m.y. “VINAYAK” on the 24™ June 2021, as she lay at the Multi- Purpose Terminal 202 (MPT 202), SALDANHA BAY HARBOUR, in order to conduct a survey to determine NATURE, CAUSE AND EXTENT, of DAMAGE TO VESSEL'S CARGO CRANES RUNNER WIRE AND PORT ROAD TRAILER, damaged by stevedores during cargo operations, DESCRIPTION OF VESSEL The vessel is a motor driven bulk cargo carrier of 58 089 metric tons summer deadweight built in 2009 in China. She has 5 holds served by 5 hatches located forward of the engine and accommodation spaces. All tank tops and bulkheads are unsheathed and no wooden or metal sparring is fitted against the ship's side framing. The weather deck hatches are served by sectional steel lids fitted with rubber gaskets, these gaskets operating against compression bars set into the hatch coamings. Access to the holds is gained through booby hatches situated on the main deck. The vessel is fitted with her own cargo working gear, which consists of 4 x 30 ton Safe Working Load Cranes. The vessel is fitted with natural ventilation. DETAILS OF ATTENDANCE According to our instructions, we attended onboard the vessel whilst she was alongside at MPT 202 Saldanha Bay Harbour. On boarding the vessel we met with the Master and Chief Officer and advised them that we were appointed by Owners P&! Club to investigate the incident. Also in attendance was Mr. Mauritzo Pisano of Bay Marine, surveyor on behalf of the Shippers and Peter Kirkman, surveyor on behalf of the Charlerers, Oldendorff. According to the Chief Officer, the vessel berthed on the 21% June 2021 and commenced cargo operations at 07:45hrs on the 22° June 2021. The vessel was nominated to load Manganese Ore in bulk using ships cranes. On the 23" June 2021 around 18:45 the crane runner wire of crane #4 parted under load and dropped the loaded cargo bin about 5 meters onto the port trailer on the quay. ainaemniy / The Chief Officer advised that the loading was not being monitored at the time but the duty officer was making regular rounds on the main deck. The Chief Officer stated that the crane wire was damaged by the stevedores when loading in the forepart of Hold #4 and the runner wire was coming into contact with the hatch coamings. The Chief Officer advised that prior to berthing in Saldanha Bay the crew performed routine maintenance on the ship's cargo gear in that the bearings were greased, and the wires inspected for damage and were greased. No defects were observed. This was being done in accordance with the Planned Maintenance Schedule. (See Annexure “A") The Chief Officer advised that the runner wire with certificate number 08/65347/042/8 was fitted on the 20" February 2021, Copy of the Certificate is attached, See Annexure “B". The last Class inspection and load test was done on the 3 June 2019 with no defects recorded, See Annexure "C”. The last Class Yearly visual inspection was done on the 4" June 2021 with no defects recorded, See Annexure “D”. The Chief Officer further advised that prior to commencement of cargo operations at Saldanha Bay, the cranes were inspected by the Stevedores and a copy of the vessel's “Inspection of Ship's Gear’ was signed and given to them, See Annexure “E”. This document also indicated the running wire operating hours, Noted to be 160hrs. According to the duty Officer the stevedores were loading the bins from the quay and lowering them into the hold where they released one end. The Stevedore crane driver would then heave on the runner wire in order to tip the bin so that the contents would spill into the hold. This practice is common. The Master confirmed that the Stevedores were placed on Notice for the damage to the ship’s gear. See Annexure “F”. According to the weighbridge, the dropped bin was weighed and given as 21.540mt. The lifting gear is approximately 2mt. Our inspection of the wire showed a damaged area of approximately 3 meters in length where all of the lays were broken. This indicated that the damaged was caused by sudden stress being inserted on the wire over a period of time. Noting the time of the incident being 18:45 only 15 minutes before the shift change where the common practice is for the stevedores to leave early, itis likely that the crane driver was in a rush to load the last bin prior to the shift change. The sudden hoisting of the bin causing a kinetic effect on the load resulted in the hoist wire becoming over stressed and parting, a Psy \ alinderniy / (= To ensure that the damage was not caused by other factors the jib head sheaves were inspected and found to be free moving with no evidence of chafing or the wire having dislodged from the sheave. We did note that the preventer guard was adrift on the port side, but this did not contribute to incident. An inspection of the coiled wire showed a number of broken single wires , however the number over the length did not meet the criteria for the wire to be replaced. We inspected the hatch coamings of hold #5 to see if the wire had come into contact with the coamings. There were traces of contact observed however none could be determined to be fresh markings. According to Capt. U. Kreitz of VHT in Germany who wrote a paper on Wire damage on Ship's cranes in 2005 and he recommended the following: According to the maintenance directives from shore-crane operators, a hoisting wire above 32mm should be discarded after 2500hrs of operation or 4.0 years. This directive is likewise recommended to ship's crane operators. Other than the damaged runner wire, no other damage to the vessel or cargo gear was sighted DAMAGES: According to the stevedore foreman, their equipment was not damaged and the spreader gear and cargo bin is suitable to continue loading operations. The port road trailer was noted to be a Three-Axel flat bed trailer with 4 steel rims and rubber inflatable tyres on each axel. All 12 tyres were noted to be still inflated, however the rims of the inner tyres were noted to be flared on the lower part. The rear axel was noted to be sheared, and the securing “J" bolts attaching the axel to. the rear springs adrift No damages were noted to the load bed. CONCLUSION: On checking the vessel's certificates and logs we noted that the stipulated inspections and test had been carried out by a competent authority. From the location of the break, we don't believe that the cause of the damage was the wire coming into contact with the hatch coamings during cargo operations. We believe \ gineraniy / the damage was as a result of the sudden hoisting of the cargo bin causing a springing effect and subsequent kinetic force on the hoist wire over time. On monitoring the loading of Manganese in other holds, we noted that the crane operator lowered the bin into the hold at a moderate speed and just before the bin reached the cargo in the hold, he reduced the lowering speed preventing a springing effect. Taking into account the weight of the Bin, cargo and lifting appliance, the weight was well within the SWL of the crane. ‘An inspection of the runner wire showed signs of wear and tear in the area of the break.
