information that New York (NY) state requires only.  

Please print and check all your documents:

Verify that the data you entered is correct on your Voter Registration/Absentee Ballot Request form
and the accompanying Addendum page.

If corrections are needed, return to the NY website, https://newyork.overseasvotefoundation.org to


either: 1) access your Voter Account and regenerate the form with corrections or; 2) if your browser
is still open with the same session running, go back and make changes, then reprint or; 3) run through
the registration program again and reprint your form.

Print, sign and date the forms:

Sign and date your Voter Registration/Absentee Ballot Request form and Addendum page.

Make sure to date the forms in U.S. format - Month/Day/Year.

Your election office needs your original signature in order to process and approve your application.
