NEGATIVE Rapid COVID-18 test, WITHOUT Symptoms:
The patient is without symptoms concerning for COVID-19 AND had a NEGATIVE Rapid cee ‘test in office AND has not
hada high-risk exposure to COVID-19, Patient may to return to work/school (or as per thelr policy)

Patient had a NECATIE Rapid COVID-19 test in office AND has not had a high-risk exposure to COVID-19, Because patient
‘was symptomatic, a confirmatory PCR swab was sent to an outside lab, Patlent should self-quarantine until they receive this
PCR test result,
‘* Ifthe PCR Nasal Swab test is NEGATIVE, the patient may immediately end quarantine and return to
work/schoo! without restrictions.
* Ifthe PCR Nasal Swab test is POSITIVE, the patient should isolate at least until (date) which 10
days after onset of symptoms _ (date). They may then return to work/school (or as per their
policy), provided they are fever-free for 24 hours, AND they are released by their local DOH.
G_ NEGATIVE Rapid COVID-19 test, with HIGH-RISK EXPOSURE (within 6 ft of COVID (+) Pt > 15 min total over 24 hrs):
ASYMPTOMATIC (no current symptoms): The patient is without symptoms concerning for COVID-19 and hada

ASYMPTOMATIC + COVID vaccine: Patients without symptoms and has received the 2% dose of the COVID-19
\VACCINE more than 14 days ago (and within the past 90 days) and does not need to quarantine after their recent
high-risk exposure (provided they remain asymptomatic). The patient must monitor for symptoms and should
