Nurse : Good morning, Maam.

Patient : Good morning, Nurse

Nurse : How are you today

Patient : I’m good

Nurse : okay, my name is karina. I will be your assisting nurse for today. Can you tell me your name?

Patient : Amalia

Nurse : can you state your date of birth?

Patient : 6th of February, 2000

Nurse : okay, I’m here to help your oral care. is that okay?

Patient : yes

Nurse : I am going to start with question. Do you have any problems with your gum or tooth?

Patient : no

Nurse : okay, it’s time to brush your teeth. Do you need my assistance to brush your teeth?

Patient : yes, thank you.

Nurse : I’m going to get your bed head up in 90 degree angle

Patient : yes

Nurse : I’m going to use my towel is a barrier and place it across best chest

Patient : okay

Nurse : I’m going to move the table in front of bed and start by giving her a small sip of water to
moisten your mouth.

Patient : okay

Nurse : I will apply toothpaste to the toothbrush and begin brushing your teeth. I use a front to back
motion to clean the outside surfaces of the teeth to clean the chewing surfaces of the teeth and
again to clean the inside surfaces of the teeth. I’ll also make sure I use a front to back motion to
 clean your tongue, I’ll move to the front and use an up and down motion to clean the front of
the teeth. After the teeth are clean. I will offer the emesis basin, so you can spit out the extra
toothpaste. I will offer water so that you can swish and spit.

Patient : okay

Nurse : next, I will take a piece of dental floss that will be approximately is inches long.

Patient : okay

Nurse : I will wind it on my fingers and I will floss in between two your teeth and then I will move down
the dental floss

Patient : okay

Nurse : floss, floss,shift (2x) until I flossed in between all of your teeth.

Patient : okay

Nurse : next, I will offer a small sip of water to rinse your mouth again.

Patient : okay

Nurse : and then I will offer your mouth wash solution with one part of mouthwash to three parts of
water and I will allow you to rinse for approximately 30 second.

Patient : okay

Nurse : after your mouth is rinsed. I will use a corner of the towel to pout your face dry and I will offer
be you lip balm as long as you’re not on oxygen. Would you like me to apply this lip balm?

Patient : yes nurse

Nurse : I will then remove my supplies and remove your towel the towel will go into the my laundry
hamper. I will return the bed back down to 30 degrees with a side rails app put the bed in low
position and make sure that bed has your call light and reach

Patient : okay

Nurse : how’s that Mrs. Amal?

Patient : okay, thank you
