Termination of Lease- 4038

I, M.M.B.J.W. Kumara, bearing NIC 931320467V, who signed as the lessee in the agreement 4038
which was attested by M.R.D. Perera- Notary Public of Colombo, hereby confirm that I am vacating
the premises on 2nd July 2021 as mutually agreed with the heirs of late Mr. John Charith Arjuna
Kurukulasuriya. Further I hereby confirm that I have no claims or liabilities against the subjected
estate. The property keys are being handed over to Ms. Dini Kurukulasuriya whom has signed below.
