AFFIDAVIT OF CORRECTION

That I, BON JOVI, of legal age, Filipino, married and with principal office
address at _________________________ after having been duly sworn to in
accordance with the law do hereby dispose and say:

1. I am the duly authorized representative of ____________ and executed the

Affidavit of Consolidation dated 17 November 2020 FAO: Ronald Mc Donald
pursuant to the authority granted to me under Board Resolution No. 12345 dated
November 28, 2003.

2. It appears that there was an error on paragraph 3 pertaining to the Entry No. on
the above-mentioned Affidavit of Consolidation. Instead of Entry No. 2019xxx, it
should be Entry No. 2019xxx.

3. That I am executing this Affidavit in order to request the Honorable Office of the
Registry of Deeds of Calamba to facilitate the consolidation of the title in favor of
ABC Bank and for whatever legal purpose it may serve.
