I, JOSEFINA, married, Filipino, of legal age, and with postal address at

_______________________, after duly sworn in accordance with the law, depose and states
that:

1. I have been using the name JOSEPINA in all my personal dealings and
transaction as evidenced by all the government issued identifications in
my favor.

2. I am legally married to JOVENCIO on 30 February 2002.

3. However, when I requested for a copy of my birth certificate with the

Philippine Statistics Authority, I have discovered that my name was
registered as “DIOSALINA” born on 31 July 1978 by parents DAMAZO
and EPIFANIA, instead of “JOSEFINA” which I was accustomed to use
since childhood.

4. For confirmation purposes and to remove the confusion, I am confirming

that DIOSALINA as registered in my birth certificate and JOSEFINA as
appearing in my personal dealings and transactions as evidenced by all
the government issued identifications in my favor refer to one and the
same person.;

5. That I have executed this affidavit to attest to the truth of the foregoing,
and for whatever other purpose it may serve.
