Field Visit Report (YLK-1)

Visited DGM-Production, CE-Drilling and Safety Officer (Electrical) on 01.11.2019 at YLK#1.

The following points are noted / attended at YLK-1:


1. Ensured no rain water is accumulated inside the cellar pit.
2. Bleed off port of Section - D well head - Noticed gas leakage from this port during field visit
on dated 01.11.2019. The said leakage was arrested by putting Teflon tape with screw.
Picture is attached in Annexure.
3. Well was prepared for bleed off after ensuring the bleed off line and pressure gauge.
4. Recorded annulus pressure= 1500 psi prior to bleed off.
5. Started bleed off @ 12:00Hrs. External gate valve in bleed line was kept open less than
10%.
6. Well pressure was very slowly dropped down to 200psi from 1500psi @ 15:30Hrs.
7. Bleed off operation was stopped @ 15:30 Hrs. Only gas was observed during bleed-off.
8. All manual isolation valves (4 nos of valves at Christmas tree and 1 number at bleed line)
are closed firmly.
9. Temporarily barricaded the cellar pit with caution tape. However, DGM-Civil to take
permanent solution for the above.
10. Working platform/floor gratings needs to be provided on cellar pit for operating gate valves/outlet
valves of well head.
11. Measured the composition of bleed-off gas with Multi-gas detector, found the LEL%-High, CO=
32ppm.
