VOLUNTARY ANIMAL SURRENDER

I, Lorna dela Cruz of legal age and a resident of #45 Pisa St. BF Thai BF Homes Paranaque
City hereby depose and state that:

1. I have been the custodian of the stray cats in my village for quite some time now
and because I am already 82 years old, I am having a hard time keeping up with
the maintenance of the kittens.
2. I hereby surrender any and all my rights over the animals to the Paranaque City
Veterenary Service Office.
3. To my knowledge, no other person has any right to these cats and I understand
that by surrendering my rights, the Paranaque City Veterenary Service Office has
all the legal rights to the animals.
