This is to inform you that Alexander Bonife is a bonafide ABC employee is in good health
and fit to work. Mr. Alexander Bonife has not visited any hospitals in the past 60 days and
have not travelled outside Metro Manila. He is also not exposed to any person suspected or
confirmed covid-19 patient. Lastly they have not been exposed to any farm or wild animals.

The information I have given is true, correct and complete. I uderstand that failure to
answer any question or giving false answer can be penalized in accordance with law.
