1. I/we authorize UnionBank of the Philippines (“Bank”) to notify me/us, through text or e-mail messages, of any and all reminders, notices, marketing materials,
promotions, loan offerings and all other Bank offerings and product updates. I/we understand that the Bank may use my/our personal information to pre-qualify
me/us for other product offerings. I/we confirm that I/we am/are solely responsible for the security of my/our cellphone and e-mail and thus here by hold the Bank

and share said basic data with other lenders authorized by the CIC, credit reporting agencies duly accredited by the CIC and financial service providers. I/we
consent to the processing, sharing and/or transferring of my/our Personal Data as this term is defined under Republic Act No. 10173 otherwise known as the
“Data Privacy Act of 2012”, its Implementing Rules and Regulations and all other issuances of the National Privacy Commission and hereinafter referred to as the
“Applicable Date Privacy Regulations”, or otherwise, relating to the loan/credit facilities that I/we are applying for and/or our account(s) without need of notice,
to (i) government regulatory agencies, credit information/investigation companies, financial institutions, credit bureaus, other banks, credit card companies,
loyalty program partners, consumer reporting or reference agencies such as Credit Card Association of the Philippines, Bankers Association of the Philippines,
Credit Information Corporation, and the Transunion Credit Bureau for the purpose of credit investigation, consumer reporting, or for reports of my/our credit
history and account updates; (ii) third persons, correspondent banks, service providers and entities, as the Bank deems necessary, to enable the Bank to service
my/our account/s and to provide all the existing features of my account/s, the Bank in the processing of my/our data, including its collection, recording,
organization, storage, management, protection, updating or modification, retrieval, consultation, use, consolidation, blocking, erasure or destruction ; (iii) third
parties engaged by the Bank or by its subsidiaries or affiliates for the purpose(s) of direct/ indirect marketing to offer selected products and services which may
interest me/us unless specifically instructed otherwise in writing; (iv) insurers, insurance brokers or providers of deposit or credit protection or protection against
all kinds of risk; or (v) such other persons or entities the Bank may deem as having authority or right to such disclosure of information as in the case of regulatory
agencies, governmental or otherwise, which have required such disclosure of information as in the case of regulatory agencies, governmental or otherwise, which
have required such disclosure from the Bank. I/we agree to hold the Bank, its affiliates, subsidiaries and third party service providers free and harmless from any
liability that may arise from, or in connection with, the collection, processing, use, storage, updating and transfer/ disclosure/ sharing/ communication of my
personal information pursuant to, and in compliance with the consent given by me/us under this Agreement.

3. For Credit Cards:


a. I/We understand and accept that my failure to provide true, complete and correct information toUnionBank of the Philippines or its authorized representative/s
regarding my/our financial knowledge and experience may result in UnionBank of the Philippines’ inability to accurately determine the appropriateness of a
product and/or service for me/us given the limited information available. I/We understand that should my application be denied, UnionBank of the Philippines or
its authorized representative/s has no obligation to provide me/us with the reason for such rejection.
b. I/We hereby agree that depending on the assessment of my/our application, the Bank will only send me/us the Card/s I/we are qualified for.
c. I/We authorize UnionBank of the Philippines or its authorized representative to, pursuant to MORB Section 301 GRANT OF LOANS AND OTHER CREDIT
ACCOMMODATIONS as implemented by BIR Revenue Regulation RR-4-2005, conduct random verification with the Bureau of Internal Revenue in order to
establish authenticity of the Income Tax Returns (ITR), accompanying financial statements and such other documents/information/data submitted by me/us.
d. I/We hereby agree to be bound by the Terms and Conditions of credit card application promos (whenever applicable) and the Terms and Conditions governing
the issuance and use of the UnionBank Credit Card.

4. For Auto Loans:


a. I/We certify that the information provided in the application and all supporting documents are accurate and verifiable.
b. I/We authorize the Bank to conduct checking on the accuracy/correctness of all information including loan and deposit checkings contained in the application
form and all supporting documents.
c. I/we certify that the full price or cost to the consumer including all interest, fees, charges, and penalties has been disclosed. It states whether interest, fees,
charges, and penalties can change over time. The method for computing said interest, fees, charges, and penalties are disclosed consistent with existing
regulations. Rates, fees and charges may vary depending on the date of application. Client accepted rates and charges may be confirmed upon signing the
Customer Acceptance Form.
