For Funeral Benefit

 Duly accomplished Form for Funeral Benefits

 Death certificate of Member – Local Civil Registrar
 Birth Certificate issued by PSA or two (2) valid government issued IDs with date
of birth and signature
 Official Receipt of Funeral Expenses issued in the name of claimant (original
copy)  

For Death Claim & Survivorship Benefits

 
 Duly accomplished Application Form for Retirement/Separation/Life Insurance
Benefits
 Duly accomplished Application Form for Survivorship Benefits
 Service Record (indicating the last day of service & date of separation)
 Death certificate of Member –Philippine Statics Agency (PSA) or Local Civil
Registrar (LCR)
 Death Certificate of Parents _ PSA / LCR with OR
 Birth Certificate of all Brothers & Sisters –PSA / LCR with OR
 Marriage Contract of all Female Sisters – PSA / LCR with OR
 2 valid Id cards of all Brothers & Sisters
 Proofs of Surviving Legal Heirs
