PERSONAL INFORMATION-MEDIA COLLECTION -TESTER CONSENT

I _________, residing at _______________, hereby acknowledge and agree to the following as a condition of
my participation in the testing by Applause App Quality, Inc. (including through its “uTest” testing division,
and hereinafter referred to as “Applause” or “Contractor”) of their customer’s personal information submission
project (the “Testing”):

I hereby acknowledge and agree that (i) I am at least eighteen (18) years of age and am voluntarily
participating in and consent to the above-referenced Testing; (ii) that I have no medical conditions and/or am
not under the influence of any drugs that could impair my ability to participate in this Testing, and (iii) that I
am voluntarily assuming the risk of any damages, injuries, claims or death relating to my participation in the
Testing.

Testers must explicitly consent to participate in the video recordings and images or other Media (“Media”) by
signing this Agreement.

Description of Activity

Testers shall perform their Media collection and uploading activities from private locations. Testers will
possess the necessary devices for Media capture, and uploading (such as mobile phones, desktops, etc.).
Testers will provide the collected Media directly to Contractor.

Contractor will provide notification to each Tester prior to that Tester being permitted to work on a Project.
Contractor represents, warrants and covenants that such notification shall comply with the requirements of the
CCPA, GDPR, and all other applicable privacy and biometric laws, and shall include:

By participating in the Project and submitting Media or other content to Applause, you acknowledge and agree
that:

(1) you will only provide Media of yourself and do not include any other personally identifying information

(2) Applause and any customer of Applause to which Applause may provide the Media you submit will use
commercially reasonable methods to protect the confidentiality of any videos you submit (subject to Applause
and its customer’s right to reject and/ or delete those videos you may erroneously supply);

(3) you consent to your Media being processed, used, and for additional information to be created (including
face vectors or other information) by Applause’s and any customer of Applause to which Applause may
provide the videos to you submit; and

(4) you will not discuss or share any information related to this project with others.

(5) you will follow the testing cycle instructions prior to submission to Applause.

I hereby further agree,  on behalf of myself, my heirs, administrators, and executors, to waive, release and hold
harmless Applause and Applause’s customer,  and their successors and assigns as well as all affiliated or
related companies of Applause and/or Applause’s customer (including companies controlling, under common
control with or controlled by Applause and/or Applause’s customer), directors, officers, employees and agents
thereof (all of which are collectively hereinafter referred to as the “Released Parties”), from any and all claims
(including without limitation claims for property damage, personal injury or death) that result directly or
indirectly from my participation in the Testing. I understand that by signing this agreement, that I am waiving,

among other claims, any present or future claims I might have against the Released Parties for injuries or
damages to my property or to me arising from my participation in the Testing. I understand that this Waiver
will bar, among other claims, claims against the Released Parties arising from injuries or damages sustained by
me due to negligence while participating in the Testing, injuries occurring as a result of the acts of other
participants in the Testing, or loss of personal property in connection with the Testing. I further agree to
INDEMNIFY, HOLD HARMLESS, AND DEFEND the Released Parties in any claim, action or proceeding
from and against all liabilities, strict liability, claims, damages, punitive damages, property damage, actions or
causes of action, for or relating to my participation in the Testing, regardless of whether the act or omission
was caused in whole or in part by the negligence of one or more of the Released Parties or by hidden, latent or
obvious defects in connection with the Testing.
