I, JULIUS MAGSINO, of legal age, Filipino, with postal address at

District I, Cauayan City, Isabela, under oath, do hereby state:

1. That I am the owner of USMAG Photography Studio, located at Don

Cancillier Avenue, District I, Cauayan City, Isabela;

2. That I have caused the processing and renewal of my Business

Permit for the year 2021;

3. That as a requisite of my renewal, there is a need for a declaration

of my revenue for the previous year 2020. Sadly, by reason of the
Pandemic, my business was not able to gain any income for the
previous year and thus, hereby declare no income for the previous
year;

4. That I am executing this affidavit voluntarily and for purposes of my

application for renewal and for any other legal purpose that it may
serve.
