To determine guidelines on emergency action procedures for incidents and emergency situations for preventing and mitigating the likely ines and injury. 1.2. To.assess all potential hazards and threat related to Tanjong Kling Power Station (TKPS) operation. 13. To determine and prepare for all emergency situations to prevent loss of human life, minimize injuries, reduce asset and environmental exposure, optimize loss control and restore to normal operations smoothly. 2 SCOPE. 2.1 The instruction covers procedures and actions faken in case of Emergency. The PSM shall oversee the coordination of the ERP to ensure incorporation of the management structure into the Emergency Response Committee |ERC) of TKPS. Please refer to Appendix | for KPS Emergency Response Committee. Emergency Operation Director (EOD) The EOD position shall be taken by the PSM. The EOD has the overall responsibilities and control of the Emergency Response Team (Team). The EOD also ore responsible in: a. Ensure the emergency response procedure are executed and implemented during emergency situation. b. Reporting of any emergency situation fo CSO and HQ. ©. Evaluate disaster vulnerability. Emergency Coordinator (EC) The EC roles shall be carried out by the plant Senior Maintenance Manager. During his absence, the EOD will nominate personnel to fil in the EC roles. During off working hours, the Shift Charge Engineer shall carry out the EC roles in the absence of the Senior Operations Manager. During emergency situation, the EC shall a. Activate the emergency response procedure and ERT during emergency situation. b. Ensure the ERP and emergency response procedure ore updated. c. Assist EOD and coordinate the emergency situation from the Emergency Control Room. d. Maintain communication with ERT and extemal emergency agencies. ‘©. Coordinate ERC members during emergency evacuation. f. Coordinate with ERC members on the maintenance of all emergency equipment. Fire Emergency Coordinator (FEC) The Mechanical Engineer with the assistance of Safety & Security Executive (SSE) will cary out the duty of Fire Emergency Coordinator |FEC) os the following: a. To facilitate and coordinate any emergency situation involving fire b. Maintain communication between Emergency Coordinator and Fire Department. . Spillage Emergency Coordinator (SPEC) The Chemical Services Executive will cary out the duly of Spillage Emergency Coordinator (SPEC) as the following: a. Coordinate spillage related emergency b. Maintain communication between Emergency Coordinator and relevant external agencies. ©. Identify all spillage conirol equipment and ensure the equipment are in good condition. d. Coordinate with emergency response team (ERT) and provide assistance to external agencies for spillage contral. e. Ensuring correct installation and maintenance of all spillage Control equipment (bunds, sumps, drainage pumps, valve, oil water separators, absorbent pads and sand bags.) f. Identify and coordinate to prepare any required PPE for chemical spillage control. 6.15. Gas Emergency Coordinator (GEC) The Mechanical Manager shall cary out the duty of Gas Emergency Coordinator (GEC) as the following: a. Coordinate gas related emergency. b. Act as main contact point between TKPS and Petronas. ¢. Maintain communication between Emergency Coordinator, Petronas Gas and relevant external agencies. d. Coordinate restoration and repair of gas delivery system. e. Coordinate with emergency response team (ERT) and provide assistance to extemal agencies for gas related emergency situation. t. Ensuring correct installation and maintenance of all gas emergency control equipment (shut off valves, pressure readers etc.) a. Responsible for checking and monitoring gas skid system and gas control equipment. 6.1.6 Security Emergency Coordinator [SEC) The Safety & Security Executive (SSE) shall cary out the duty of Security Emergency Coordinator (SEC} as the following: ‘a. Coordinate security related emergency. b. Maintain communication between Emergency Coordinator, TKPS security team and Police Department, c. Coordinate the emergency evacuation process. d. Ensuring correct installation and maintenance of all security control equipment (CCTV, fencing, alarm etc.) 7 PROCEDURE. 7.1 Emergency Preparedness. 7.1.1 Policy TKPS management shall practise and maintain continuous planning to minimise the danger to life, health and property from emergencies or disasters; cooperate with local authorities and agencies in charge with disaster control and take necessary and prudent steps to assure continuity of operations, or restoration of production and distribution activities as quickly «@s possible following an emergency or aisaster. 7.1.2 General Threat Assessment @. Internal Threats An internal threat is any action which jeopardises site security and safety caused by an intemal system, employee, or contractor, over which the Station has direct control. Examples of events in this category, which could lead to an emergency situation, include the following: * Structural collapse of plant or work platforms causing substantial damage and injury. * Major technical failure, possibly caused by an explosion, resulting in substantial damage and threatening life. * Disruption of operations and damage to plant and personnel caused by hostile industrial action, * Sabotage. b. External Threats ‘An external threat is any action which jeopardize site security and safety as a result of human external activity over which station management has no direct control. Examples of evenis in this category, which could lead to an emergency situation, include the following: * Violent protest action fuelled by emotive political, business or environmental events or circumstances. * Bomb threats. ‘PAHLAWAN POWER SDN. Emergency Situations and Notification Emergency situation is defined as a situation, actual or imminent, involving unusual conditions and hazards which it not corrected or prevented could lead to a disaster. A nofification code is a code indicating the actual or probable seriousness of any disaster or emergency situation. There are 3 types of codes for emergency notification as follow: Code type _| Definition Code 1 Possible emergency. No immediate danger, with estimated 3 hours or more lead time {latency between the initiation and execution of a process) Code 2 Probable emergency. Danger apparent, with estimated 3 hours or less lead time Code 3 Immediate emergency. Disasterimminent with little or no lead time. The following table shows the actions to be taken for each of the emergency codes. This is a temporary organisation consisting of key station personnel with extemal agencies assistance as required. The ERC shall be led and under operational direction of the PSM Psy % or an appointed Deputy. The Emergency Coordinator, namely ey aS the Shift Charge Enginees, shall be responsible for on-site (s// necene coordination of FIRE, SPILLAGE, GAS & SECURITY EMERGENCIES. 5 208 | , OF NS w During a major emergency situation that required assistances eo from extemal emergency agencies, the PSM as the EOD will stay Deere in command until being relief and take over by the Senior Cries Officers of the external agencies involved. The PSM or Deputy, will continue to provide assistance to the external emergencies agencies by providing technical advice, facility control, and available resources. 7.2 Emergency Response. 7.2.1 Emergency contact list for Pahlawan Power Sdn Bhd. Please refer to Appendix 3 and Appendix 4 for main emergency contact numbers for local authorities/ agencies and key personnel for Tanjong Kling Power Station respectively. 7.2.2 Evacuation The decision to fuly or particlly evacuate the site, is to be made either by the Shift Charge Engineer, Emergency Coordinator or Emergency Operation Director depending on hierarchical availabilty. Area supervisors are to be responsible for the evacuation of all personnel in their care. They will be assisted as PAHLAWAN POWER SDN. BHD. Security personnel are to provide the Emergency Coordinator with a list of the names of those available for redeployment, based on attendance list. In selecting evacuation routes and assembly areas care must be taken fo avoid obstructing the fire brigade and other emergency services. Consider wind conditions and the way smoke or gas would probably travel, and ensure they are well clear of shattering glass or other secondary shrapnel. On site personnel should be accounted for. The area supervisor with the assistance of security personnel shall camy out area sweeping after all personnel has evacuated. A systematic method shall be used for the headcount for all personnel. 7.2.3 Command and Control Center (CCC} A designated area shall be established for the emergency control room and casualty area. The primary Emergency Control Room (ECR) and casualty area is at the First Aid Room on the ground fioor of the Administration Building of the power station located south west of TKPS, Alternative ECR and casualty area is to be in the Guard House of the station located North-west of - ‘TKPS. y O F recone \ The Emergency Response Committee (ERC) should hold all routine meetings at the control centre during emergency situation. The ECR should be equipped with permanent PC to mae monitor and display any relevant data [including all contact details of personnel not included in the key personnel call list) and graphical information needed fo control a Fire, Spilage and Security Emergency. The main purpose of the display is to have as much information as may be needed by those controling an emergency and is openly available for immediate reference and use. Apart from the visual display, the ECR also shall be equipped with @ large scale layout plan and topographical information for emergency response planning and rescue purpose. PAHLAWAN POWER SDN. BHD. Laminated copies of the Emergency Action Sheets 3 hall be made available at all strategic areas. Please refer to ‘Appendix 2 for details on the Emergency Action Sheet. 7.2.5 Emergency Dri 7 ye he plant shall conduct emergency drills minimum 4 times a ear as required by Edra Group Safety. The purpose of the drill is jo test the effectiveness of the emergency response to a defined scenario. These tests can concern the whole emergency procedure, or only part of it (notification process, responsibi fies awareness, rescue operations). The drils will not be necessarily communicated in advance to the personnel. Fi y ull plant evacuation drill shall be performed at least once a ear at the most relevant period. After each drill, an experience retum feedback shall be written by the Emergency Response Committee and communicated to all plant employees by any means. As a result of this feedback and as necessary, the er mergency Response Plan and procedure shall be updated. 7.2.6 Emergency Response Committee Meeting The Emergency Response Committee (ERC) shall meet _on regular basis at a suitable time that to be determined. The purpose of these meeting are as follows: To confirm the validity of all data and effectiveness of all equipment/ procedures in the Emergency Control Room (ECR). To conduct incident and defect analysis on all protection systems based routine report. The generated reports should be specific to the fire protection systems, and engineers should look for performance pattems that may assist with preventative maintenance or signal the need for a replacement program. To establish an on-going test program for all protection equipment and monagement systems, and to witness the same. To establish and monitor program at ail levels cimed at increasing safely and preparedness for general and major emergencies on site. To establish and monitor @ regular on-site training regime. Mass automated dialing services such as Reverse 999, as well as common siren systems that are used to alert for Fire, tsunamis, airraid, and other such incidents, are examples of emergency notification systems. * Emergency communication systems offen provide or integrate those same notification services but will also include two-way communication—typically to facilitate communications between emergency communications staff, affected people, and fist responders. Another distinguishing attribute of the term “communication” may be that it implies the ability to provide detailed and meaningful information about an evolving emergency and actions that might be taken; whereas “notification” denotes a relatively more simplistic one-time conveyance of the existence and general nature of an emergency (such as Emergency Rescue Location). 8.0 EMERGENCY SITUATION SCENARIOS The provision of this plan must be carried out immediately whenever there is ‘emergency situation such as: 8.1 Fire 82 Accident and injury 83 Confined space accident 84 — Leak or spill of hazardous materials fo the air, soil or water which could threaten human health or the environment, such as gas, fuel oil or chemical substances 85 Geophysical disasters such as flood, heavy thunderstorm, haze or Tsunami 8.46 — Bomb threat or civil disturbance e.g. riot PAHLAWAN POWER SDN. Even though the diesel has high flash point, it may have caught on fire if it is exposed to the naked fire. A heavy thunder storm warning is issued when the winds of 74 mph or higher, or a combination of dangerously high water and rough seas, are expected in the area within 24 hours. Once a hurricane watch has been issued: + Stay calm and wait insiructions from the Emergency Manager. Secure all equipment tightly, or move to a safe place if time allows. Continue to monitor local TV and radio stations for instructions Move early out of low-lying areas.
