It is with great reluctance that I am writing this letter to inform you that the Library
Identification Card, together with the Borrower’s Card, that I shall present to you
during library use and the signing of clearance were affected by the fire during the time
when the house of my grandmother located in Sitio Narra, Malitbog, Bongabong
Oriental Mindoro was burned down on February 15, 2020.

Regarding this case, I, Karlo M. Aranda, a third (3rd) year student from Bachelor of
Science in Information Technology (BSIT) of Mindoro State University –
Bongabong Campus, am requesting on your good office for another issuance of
Library ID and Borrower’s Card on my behalf to continue using your library services.

I am hoping for your favorable response and utmost consideration on this request.
