What is her diagnosis?

Bipolar Disorder
What are the symptoms that helped you make this diagnosis? What diagnostic criteria do they
relate to?
There are diverse signs which have brought about the suggestion that Mrs. Spinner is having
bipolar disorder. First, this lady is attacked by dizziness severally, a condition which makes her
become incapacitated. Moreover, she experiences a combination dizziness and nausea which
occurs 4-5 times in every week. Furthermore, when Mrs. Spinner gets exposed to ‘shimmer’, she
feels floating hence being unable to maintain her balance and being unable to sleep (Geddes &
Miklowitz, 2013).
What are two other possible diagnoses and why did you not choose them?
The other diagnoses which Mrs. Spinner appeared to have included vestibular neuritis and
schizophreniform disorder. However, there exist various reasons which made me not to choose
them. For example, despite vestibular neuritis being relating to nausea, I realized that this
condition occurs from 1-7 days, whereas our patient’s circumstance was lasting for maximum
4hrs. On the other hand, apart from schizophreniform disorder getting diagnosed through signs
such as dizziness and nausea, Mrs. Spinner ailing did not constitute hallucinations, delusions,
disorganized speech, and absence of emotional expression (Geddes & Miklowitz, 2013).
How would Mrs. Spinner respond to psychotherapy designed to address her "emotional"
problems?
Mrs. Spinner should address four types of psychotherapies so as to address her emotional
challenges. First, she should adhere to behavioral therapy thus reducing stress. She should
moreover address cognitive therapy hence being able to determine and modify thinking
schedules concerning mood shifts. This lady must also employ interpersonal therapy to be able to
mitigate strains that disease may place on her. Lastly, it is essential to employ social rhythm
therapy to be able to get normal sleep pattern as well as her daily routines (Geddes & Miklowitz,
2013).
What therapeutic approach would you consider and why?
The therapeutic strategy I would recommend for treatment of Mrs. Spinner’s conditions is a
combination of valproate and antipsychotics. The reason behind choosing this strategy is that the
patient is having severe mixed episodes. In particular, the antipsychotics to be used include
risperidone, aripiprazole, and olanzapine (Geddes & Miklowitz, 2013).
What is her prognosis?
There are diverse factors which can cause the bipolar disease. On the view of environmental
factors, this illness can originate from a life event that activates temper episode on individuals
with the genetic temperament for bipolar disorder. Also, in absence of clear genetic elements,
distorted health behaviors, drug abuse or alcohol, or hormonal issues can also cause bipolar
disorder (Geddes & Miklowitz, 2013).
