To whom it may concern,

Please be informed that the bearer of this letter is employed at [COMPANY] located at [ADDRESS where
employee is going]. [COMPANY] is a [XXXXX] company that provides [ZZZZZ] which is a critical
infrastructure essential business and therefore falls within the “essential business” exception to any
emergency or shelter-in-place order.

Employees in possession of this letter have been deemed essential to our minimum basic operations
that need to be performed in the office to ensure the aforementioned services are provided and remain
up and running.

We have requested that all personnel not essential to such minimum basic operations work remotely.

For those necessary to physically report to the office, we have been instituting health and safety
protocols within the office following published CDC guidelines. Further, before leaving home to report
to the office, we are asking each employee to run through the below questions.

 Am I exhibiting any symptoms of COVID-19?
 Have I knowingly been in contact with someone who has a confirmed case or has exhibited
symptoms of COVID-19?
