It is expected that the data gathered shall provide information in assessing the learning outcomes since learning delivery has been diversified due to the pandemic and shall serve as basis for policy recommendation in the implementation of the K to 12 Basic Curriculum and other related programs and projects. 3. All submissions must be consolidated at the district-level and shall be verified by the PSDSs/District In-Charge. The electronic copy of the consolidated report from the district shall be sent to the Area EPSvr In-Charge for consolidation on or before August 2, 2021. Download the templates from the links. 2.) Fill-out the template with the data that is only available in your school. 3.) Submit the electronic copy to the district for consolidation. District Level 1.) Make sure that all schools can access the templates. 2.) Monitor the activity of the schools in accomplishing the templates. 3.) Consolidate the report from the schools. 4.)
