Application forms can be obtained from any Police Station, or through the Internet from
the Official Police website www.police.gov.cy
Every application must be accompanied by:

1. An official Receipt for the payment of the amount of €20,00. The payment could be made:
(a) remittance to the Central Bank of Cyprus in the name of the Chief of Cyprus
Police
(SWIFT CODE: CBCYCY2NACC, Government General Account, account no.
CY16001000010000000006001010) – only in Euro.
2. Identity Card, Passport, for Cypriot citizens.
3. Alien Book , Pink Slip or Yellow Slip and passport for Aliens
4. Copy of incorporation of the company, partnership, association or other entity or
association
of persons with legal personality
