AFFIDAVIT

 
            I, Melissa Ayroso, Filipino citizen, of legal age, married to Patrick Ayroso, and a
resident of (Insert Address of Affiant), after having been duly sworn in accordance with law,
hereby depose and say:
 
1. That as of January 15, 2013, M. AYROSO LUMBER & HARDWARE (single
proprietor) ceased its operations, and a new company was registered under
the name M. Ayroso Lumber and Hardware Inc. (a corporation). 

2. That the suppliers mentioned on the attachment letter of DISCREPANCY

NOTICE (DNno.) 25A-2017-00-00056, CY2017 are suppliers of M. AYROSO
LUMBER & HARDWARE, INC. (a corporation) except for LEONARDO
CORAZON L. who is not our supplier. On our assessment, most likely there
was a mix up because the two differs only on the last part, where the
corporation has an “INC.” at the end while the single prop. does not.

3. That the invoices pertaining to the purchases mentioned in the

DISCREPANCY NOTICE have been submitted to the examiner assigned to
the case.
