- Original and/or photocopy of the expired Permit of Stay and the passport used in entering the
Philippines.
- Completely filled up Application Form (Type D), with 2 pcs. passport size photo, white
background.
- Passport with a validity of not less than 6 months prior to departure.
- If you have an employer in Italy, kindly provide your employer’s complete name, address and
telephone number.

In case of expired P.S.

Notarized Affidavit of Explanation should state the following:

- The arrival date in the Philippines;

- The reason why you left Italy and returned to the Philippines;
- The reason why the Permit of Stay (Permesso di Soggiorno) was not renewed before
leaving the Italian territory;
- The reason why you came back to the Philippines and remained beyond the expiration of
your permit of stay (please provide the supporting documents);
