Dear Sir,
I, Ratul Islam, savings bank account holder of your bank with saving A/C No: 008-3201-
000022599. I lost my ATM card on 18th July at an ATM Booth of UCB. My father went to
withdraw cash from the booth but didn't complete the process. The card had stuck inside the
ATM. And I would like to highlight that I am an emigrant. I can't collect the existing or the new
ATM card by myself. Hence I want to authorize my elder brother Moinul Islam Rocky to receive
the new ATM card on my behalf.
I want to request you to issue a new ATM card and authorize my elder brother to operate my
account. I already gave him my checkbook. Allow him to withdraw the funds.
I hope you will help, grant me a new ATM card as soon as possible and authorize my elder
brother to act on my behalf. I would be very thankful to you for this kind act.
