Subject: Reimbursement of medical expenses of my delivery/ new born
Dear Sir
I, Priti Kumari (Employ id -801010), am working as an office assistant at DBGB Narayanpur,
Branch code -3620. As per the maternity benefit scheme guaranteed to an employee, I am
entitled to claim reimbursement of the medical expenses occurred during my delivery.
It is to be stated that I was on my maternity leave from 18/01/2021 to 21/07/2021 and my
delivery date was 04/02/2021. Due to certain medical complications during my delivery and due
to the ongoing pandemic, I was advised by the doctor to take utmost care of the baby. I was also
advised to take utmost precaution for the next 4-5 months for the safety of the baby. Due to this I
was not able to claim my medical expenses within a month of the delivery. I joined the office on
July 22, 2021 and therefore, through this letter, I am respectfully claiming my medical expenses
which is ₹16,000/. I am also attaching all the relevant documents and bill necessary for the
reimbursement which is best of my knowledge. Please feel free to ask for any other documents
which may require for the same.
With due respect, I humbly request you to reimburse the medical expenses as soon as possible as
it will be of great help to me. For this kindness, I will be highly grateful to you.
