1. Please complete this form in its entirety. Please then submit this form by either of the
following methods:

Important: If this form is being completed by a shop, please indicate the name of the
shop in the “Name” line and also include a personal contact name for reference.
2. Upon completion and submittal of this form, a follow‐up email will then be sent to
you indicating an approval with an issued RMA#. Please make a copy of this form and
the original invoice to be included with the returned part as the packing slip. Make
sure your full name, address, daytime phone and issued RMA# are included.
3. Pack the part(s) carefully – no rattles or shakes! Please only return items specified or
requested by the Thundermax Support Representative.
4. Do not deface or write anything on the part. No warranty will be honored on parts
that have been altered, modified, or otherwise changed from their original state.
Parts damaged due to improper packaging during the return process will not be
covered under warranty.
5. Return shipping costs are not covered by the ThunderMax Returns Process or the
original product warranty. Payment information to cover return shipping costs will be
required to complete the ThunderMax Returns Process.
6. All returns should be shipped to the following address:

7. Upon receipt of your ThunderMax, complete testing and validation will be performed.
Notably, every effort will be made to complete these processes and return your
ThunderMax unit in a timely manner. Upon completion, the ThunderMax unit will be
returned with a final letter indicating any findings and information regarding the
warranty status.
