AFFIDAVIT OF DISCREPANCY

I, ELINORA SUYU BALLESTEROS, of legal age, Filipino, married, and a resident of

Dodan, Peñablanca, Cagayan, after having been duly sworn to in accordance with law,
do hereby depose and state:

1. That my full name is ELINORA SUYU BALLESTEROS, born on 13 December 1974, and
the fact of my birth was duly recorded before the Office of the Local Civil Registrar of
Peñablanca, Cagayan. Attached as Annex “A” is the copy of Certificate of Live Birth;

2. However, when I got married with RENE N. BALLESTEROS, my date of birth that
appeared in our Certificate of Marriage was 18 December 1974 instead of 13
December 1974. Attached as Annex “B” is the copy of the Certificate of Marriage;

3. That said incorrect entry in our Certificate of Marriage was only inadvertently
entered as such;

4. That I am executing this affidavit to attest to the truthfulness of the foregoing

narration of facts and for any legal purpose.

IN WITNESS WHEREOF, I have hereunto affix my signature this 15 th day of May
