AFFIDAVIT OF CONSENT

I, PRUDENCIA D. EDAÑO, Filipino, of legal age, and a resident of Victoria,

San Remigio, Cebu, after having been sworn in accordance with the law hereby
depose and state that:

1. I am the grandmother of Tricia D. Edañ o, who was born on October 9,

2002.

2. I am the one who takes care and has actual custody of Tricia D. Edañ o.

3. Her mother regularly sends financial support for the necessities of her
daughter.

4. Her mother, Merlyn Lewis, who is my daughter, is desirous to take Tricia

D. Edañ o to her residence in Syney, New South Wales, Australia.

5. I hereby give my consent that my said granddaughter Tricia D. Edañ o to

travel with her mother Merlyn Lewis to Syney, New South Wales,
Australia, all expenses relating to or arising from the said travel to be
borne solely by my said daughter Merlyn Lewis.

6. I was not forced nor was given any pecuniary consideration in executing
this affidavit.

7. I am executing this affidavit to attest to the truth of the foregoing facts and
for whatever legal purposes this may serve.
