SAFETY TOPIC WEEK 10, 2009

LOTO (LOCK OUT TAG OUT )


Many industrial accidents are caused by the unexpected energization
or start up of machines or equipment or by the uncontrolled release of
energy.
To perform service, maintenance and Pre Commissioning work on
equipment/system safely, we must understand the importance of
energy control and how to apply energy isolation and
LOCKOUT/TAGOUT.
ACTIVITIES REQUIRED LOTO:
MECHANICAL ISOLATION
MECHANICAL ISOLATION
Energization, Confined space, All Pre Commissioning activities.
ELECTRICAL ISOLATION:
Rotary equipment alignment, Termination, Glinding etc.
STEPS TO LOTO:
• JHA for LOTO shall be prepared. ELECTRICAL ISOLATION

• PICW (Isolation Officer - Electrical/Mechanical) shall generate the LOTO permit.


• Isolation Officer (Sub. Contractor/TSK shall ensure the isolation points, Locks
and tag.
• LOTO permit shall be taken while isolating the system first time.
• LOTO certificate shall be taken while carrying out isolation in downstream.
• PA shall ensure the LOTO requirements.
• PIA shall check LOTO compliance and issue the LOTO permit.
TSK GENERAL REQUIREMENTS:
• TSK Trained and authorized personnel shall carry out Isolation.
• TCM – Blue Color lock,
lock SKEC – Orange Color lock.

• Only authorized employee who install LOTO shall remove the LOTO.
• Tag must be legible, displayed in multi lingual.
• Tag must be securely attached.
TCM
• LOTO process shall be inspected/monitored.
• Do not disturb any Lock/Tag.
• Before energization ensure safety of personnel and system.
