AFFIDAVIT

I, MARY ANNE S. MUNGCAL Filipino, of legal age and resident of Lamintak Sur, Medellin,
Cebu, after having sworn to in accordance with law do hereby depose and say:

1. That I was registered as proprietor with the BIR with TIN 316-364-759-000.

2. That I was not able to start operation of the business registered with the BIR and I am
currently applying to close the business registration with the BIR.

3. That the application of closure of the BIR has required me to produce closure certification
from the Municipality of Medellin, as a proof that I no longer/have not started operations on
my registered business.

4. That upon my application for the required Certification from the Municipality of Medellin I
was asked for permits and documents as requirements for such certification.

5. That since my business was a non-starter, I was not able to secure business permits and
documents including:
a. TOP Receipt
b. Tax Clearance
c. Location Clearance

6. That I have not started operations and have not secured that aforementioned business permits
and relevant documents, hence I cannot provide the documents as requirements.
