ZAMBOANGA SIBUGAY, after having duly sworn to in accordance with the law of the
Philippines, do hereby depose and state the following to wit:

That I took my medical examination thru ST. PETER-PAUL MEDICAL CLINIC, INC. 118
GREY ST. ERMITA MANILA and the result was pending in

- CLASS B CONTROLLED HYPERTENSION

That my principal/employer MR. TURKI MOHAMED SAAD ALFULIH was informed

and still willing to hire me despite of my medical findings.

That ST. PETER-PAUL MEDICAL CLINIC, INC shall not be held liable for any damages
arising from the particular findings, including all repartition charges and expenses
related to it, including GCC penalties.
