Dear Mr BJ,

Please be advised that a serious incident has occurred today in the FSL 2.1 & 2.2 elevator shafts.

The drain pipe inside the shaft was flowing water in to the shaft as per the attached video.

Our engineers have already informed to HKN about the drain pipe and to take necessary actions to close it.
However this was not considered and as a result today’s incident has occurred.

Note that Pit equipment of the elevators could be affected due to the water seepage therefore kindly take actions
to help rectify this issue and please advise necessary persons to cover the drain pipe urgently.
