AFFIDAVIT

I, Smt. Archana Pramanik, aged about-35 years, W/O-Sri Nakul

Pramanik, resident of Village- Durgapur, P.O-Damodarpur, P.S-Junput

Coastal(Contai), Dist-Purba Medinipur, Pin -721401, By faith- Hindu, By

occupation-Household Work do hereby solemnly affirm and declare as

follows:-

1. That I am the permanent inhabitant in the aforesaid locality within

the jurisdiction of Your notarial area.

2. That I am the citizen of India by birth.

3. That my minor daughter namely- Joyoshri Pramanik( Date Of Birth-

18/03/2009) D/O-Nakul Pramanik is a 100%(Hundred percent) physically

challenged girl having disability certificate bearing no-1058 dated

23/11/2016.

4. That my said daughter has a savings bank account bearing A/C no-

39840710460 at State Bank of India ,Nayaput Branch.

5. That my said daughter cannot operate the above said account due to

her disability and she cannot sign anywhere .

6. That I am being the mother of my said minor physically challenged

daughter want to join the aforesaid savings account as a joint operator.

7. That I am not concealing any truth in respect of my above statements

and I am swearing this affidavit in support of my above statements for

the purpose of operating the above said savings account.

Verification

Statements made above are true to the best of my

knowledge and belief . And nothing materials

has been concealed there , if so I will be

punished U/S-193 of the Indian Penal Code.
