Mode of Action
Direct renin inhibitor. Decreases
plasma renin activity (PRA),
inhibiting the conversion of
angiotensinogen to angiotensin I,
decreasing the formation of
angiotensin II.
Uses
Treatment of hypertension (not
recommended as initial treatment).
May be used
alone or in combination with other
antihypertensives.
Side effects and Adverse Reaction
Side effects
Rare (2%–1%): Diarrhea,
particularly in women, elderly
(older than 65 yrs),
gastroesophageal reflux, cough,
rash.
Adverse Reactions
CNS: Dizziness, fatigue, headache,
seizures
CV: Hypotension, peripheral edema
EENT: Nasopharyngitis
GI: Abdominal pain, diarrhea,
dyspepsia, gastroesophageal reflux
GU: Elevated blood creatinine,
renal calculi
HEME: Decreased hemoglobin and
hematocrit
MS: Back pain
RESP: Increased cough, upper
respiratory tract infection
SKIN: Rash, Stevens-Johnson
syndrome, toxic epidermal
necrolysis
Other: Anaphylaxis, angioedema
of head
and neck, elevated creatine kinase
or uric acid level, gout,
hyperkalemia

Nursing Responsibility
Administration
 Give consistently with or without
food, but not with high-fat foods.
Patient monitoring
 Stay alert for signs and symptoms
of renal dysfunction, especially in
patients receiving NSAIDs.
 Monitor patient for signs and
symptoms of angioedema;
discontinue drug if signs and
symptoms are present.
Patient teaching
 Instruct patient to take drug
consistently with or without food,
but not with high-fat foods.
 Instruct patient not to stop drug
suddenly because doing so could
result in uncontrolled high blood
pressure.
 Advise female to tell prescriber if
she’s pregnant or breastfeeding
before taking drug.
 As appropriate, review all other
significant adverse reactions and
interactions, especially those related
to the drugs, tests, and foods
mentioned above.
