Ductal Ca.

in situ

- The risk of recurrence and invasion depends upon necrosis and nuclear grade rather than the
type.

Different histological types are:

- Comedo necrosis
- Cribriform
- Micropapillary
- Solid pattern

Comedo necrosis:

- It has solid cells of tumor cells along periphery lining the ducts.
- In center there is necrosis with occasional presence of calcification.
- Tumor cells have high grade pleomorphic nuclei.

Cribriform necrosis:

- They have gland in gland appearance/cookie-cutter appearance.

- Presence of round fenestrations within the gland. The more regular the spaces are in term of
shape, size and arrangement, the more is chance for metastasis.

Micropapillary:

- There are slender elongated epithelial fronds which is solid and projects into the lumen.
- They lack true fibrovascular core.
