WAIVER

As a parent of ______________________________ I hereby allow my son/daughter

to undergo and participate the Military Science 3, under supervision of Commandant
and their Training Staff of 341st NROTCU, Camarines Sur Polytechnic Colleges, Nabua,
Camarines Sur.

Together with my child, I know that the unit, its officers and enlisted men are
expected to exercise legal diligence required for the safety and well-being of my child on
the duration and place and training. And will observe COVID-19 minimum health
protocols.

The legal diligence includes oral or written instructions whether given before or
during the training, that if followed, would ensure the safety of my child.

If my child disregards or fails to follow those instructions, or should act on his own, I
together with my child, have no claims against the unit, officer and enlisted men, should
any damage caused or liability be incurred to property or person.
