LESSON LEARNED#12 – fatal injury during lifting operations

Accident summary:
During diesel oil transfer operations on an offshore platform, the rope used to lift by the
crane the flexible hose got broken, and the hose did struck on the head an operator. The
victim did not attend the Toolbox Talk.

Main causes of the accident:


 Rope rupture due to friction with the metallic clamp of the flexible hose;
 Use of lifting materials unsuitable and not certified;
 Operator wrong position, under the suspended load;
 Excess Team confidence.

Corrective actions taken:


 Make sure that the lifting equipment is suitable, certified, checked and maintained;
 Ensure auditing, recording of incidents, and dissemination of lessons learned;
 Ensuring competent personnel to perform the work safely;
 Ensure that the lifting procedures are available to all persons involved in the work;
 Make sure that the responsibilities are clearly defined.

LESSON LEARNED
Everyone must follow the fundamental rules relating to lifting
operations, namely:
 do no pass / stand under suspended loads;
 always use certified equipment;
 always attend the Toolbox Talk.

The concept of "Stop Work Authority" can save lives:


anyone notice a dangerous situation not expected,
has the right / duty to stop the work
