AFFIDAVIT OF WITNESS

NED MANDU, an employee of Braced Lending, residing at #16

CAMARON Street, Pines City Royale, Barangay San Roque, Antipolo City,
Rizal, Philippines, 1870, duly, and respectively sworn depose and say:

1. That he is an employee of Braced Lending and has personally known

and has been acquainted to Cardi B. Dalisay, the PLAINTIFF in the
small claims case submitted to this court;

2. That to his personal knowledge, the partieds to this case have executed
a CONTRACT OF LOAN amounting to Php 100,000.00 with an
interest of 5% per annum in the event of default;

3. That to his personal knowledge, the contract of loan was evidenced by

four post-dated checks issued by the DEBTOR to the CREDITOR. That
the DEBTOR also issued a promissory note in order to assure the
CREDITOR that he is willing to comply with the terms and conditions
of the contract of loan;

4. That to his personal knowledge, the DEBTOR failed to pay for the loan
when it fell due and demandable on 25 November 2019.
