AFFIDAVIT OF DISCREPANCY

That I, SARAH KAY LUPANGO BALAJADIA, of legal age, Filipino citizen, single, with
address at 639 Challenger Street, Post Proper Southside, Makati City, Philippines, after
having duly sworn to in accordance with law do hereby depose and say:
1. That I was born on 19 June 1987 at Makati City, Philippines;

2. That in my Certificate of Live Birth my name appeared as SARAH KAY

LUPANGO BALAJADIA, the same name appears in all my valid identification
card;

3. That whereas, in the records of Commission on Elections (COMELEC), I

discovered that my name was registered as SARAH KAY LUDANGCO
BALAJADIA;

4. That in view hereof, I hereby attest that the names SARAH KAY LUPANGO
BALAJADIA and SARAH KAY LUDANGCO BALAJADIA refer to one and the
same person pertaining to myself;

5. That I am executing this affidavit to attest to the truth of foregoing facts, for
information and guidance of proper authorities concerned and for whatever
legal purpose this affidavit may serve.
