Part I: Information Sheet

Introduction
You are invited to participate in the research study on “Online Learning Modality
Effectiveness and Leaner Outcomes of Level I, II, and III Student Nurses”. You do not have to
decide today whether you will participate in the research. Before you decide, you can talk to
anyone you feel comfortable with about the research.
Below is the detailed information on what will happen throughout the study.

Purpose of the research

This main purpose of the study is to measure the effectiveness of online modality and the
learner outcomes of Level 1, 2, and 3 student nurses of Father Saturnino Urios University.

Type of Research Intervention

This research will involve your participation in answering the research questionnaire via
online using Google Form.

Participant Selection
You are being invited to take part in this research because you fit to our selection criteria
which is a student of FSUU Nursing Program and enrolled during the first semester SY 2020-
2021.
Voluntary Participation
Your participation in this research is entirely voluntary; therefore, the respondents will
not receive any payment for their participation.
Procedures
The researchers will gather information that is related to the study; conducting a survey to
collect and analyze the data and use the questionnaire as a tool. They will make a letter of
approval first to conduct the study at Father Saturnino Urios University Nursing Program, the
said letter would be sent to the Dean of the Nursing Program as well as to the student nurses who
will be the subject for the study.
After permission is attained, the study will then be conducted through letting the chosen
Level 1, 2, and 3 student nurses answer a survey questionnaire regarding the Online Learning
Modality Effectiveness and their learning outcome.
This study involves respondents to answer the Google Forms online. Once data from the
survey was collected, data shall be subject for analysis and interpretation.

Risk
This research study only involves sharing of information and online learning experiences;
therefore, no foreseeable risks are being anticipated as result of your participation in the study.

Benefits
As a respondent of the study, it will give insights on how to deal with online classes with
the ongoing COVID-19 pandemic. It will also help you to have a better understanding about your
online learning experience.

Confidentiality
The information that will be collected from this research study will be kept confidential.
Research records will be kept in locked file and all electronic information will be coded and
secured using a password protected file. The data collected will be deleted after the researchers
makes hard bound copy of this study.

Sharing the Results

The results that we get from this research will be shared with you and the researchers
involved, the panelist and the intended beneficiaries of the study. Rest assured that your name
and other personal information will not be revealed in the research report. All responses shall be
kept anonymous.

Right to Refuse or Withdraw

You do not have to take part in this research if you do not wish to do so, and choosing to
participate will not affect your study or related evaluations in any way. You may stop
participating in answering the questionnaire at any time that you wish. You also have the right to
ask questions about this research study and to have those questions answered by the researcher
before, during and after the survey.
