This is to formally request for the retirement of San Lorenzo Ruiz Investment Holdings
and Services Inc. line of business, SEO – Caregiver Training, registered under business
permit no. 15775, effective immediately.

This request for termination is due to the change in our business primary purpose.

We have already applied a business permit for the new line of business, with permit no.
15809 issued on August 23, 2021.
