1. Certificate of Lot Ownership (Xerox & Bring Original)
a. Certified True Copy of Lot Title
b. Certified True Copy of Latest Tax Declaration
c. Latest Real Property Tax Receipt
d. Lot Plan
e. Relocation Survey Plan and Report
2. In case the applicant is not registered lot owner
a. Duly Notarized written consent from the
Lot Owner/ Lease Contract/Deed of Absolute Sale
b. Photo Copy of 1a, 1b, 1c, 1d, 1e

3. In the absence of any existing Certificate of Title in the name of Applicant


a. Certified true copy of latest Tax Declaration

b. Current Real Property Tax Certification

c. Sworn Statement Affidavit to the effect that:

c.1 The Applicant is the owner/ claimant of the lot subject for application of Title
c.2 Certification from the DENR regarding the present status of the land

a. Building Plans signed and sealed by the following:
a.1 Architectural Plans (Architect)
a.1.1 Vicinity Map (half-km/2km radius)
a.1.2 Site Dev. Plan (reflect distance from RROW)
a.2 Civil/Structural Plans (Civil Engineer)
a.3 Electrical Plans (Professional Electrical Engineer)
a.4 Sanitary/Plumbing Plans (Sanitary Engineer or Master Plumber)
a.5 Mechanical Plans (Professional Mechanical Engineer)
b. Bill of Materials and Cost Estimates (signed and sealed) & Notarized
c. Structural Design and Analysis (2-storey & above) (signed and sealed)
d. Building Plans Specification (signed and sealed)
e. Specimen - photocopied ID and receipt of the Professionals (signed and sealed)
*If Electrical is 40A above (MUST BE BLUEPRINT)

For 30 m2 below
