I am writing this to you for a consideration in waiving the charges made on my account
associated with my check issued dated July 30, 2019.

My checking account was overdrawn, I have overlooked the check I issued amounting to
PhP15,000.00. It was issued three (3) months ago and I really forgot about it. This
resulted in the incurrence of fee due to insufficient funds.
