AFFIDAVIT

I, ROSE ANNE ENESIO FAJARDO, of legal age, Single, Filipino, with residence
and postal address at Bagacay, Gubat, Sorsogon, Private School Teacher at DATA
BASE TECHNOLOGY COLLEGE, INC. with GLORIA V. ACUÑA as the School
Administrator after having been duly sworn to in accordance with the law depose
and say;

THAT I am presently employed at DATA BASE TECHNOLOGY COLLEGE, INC., as

Senior High School Teacher;

THAT I am bind with a contract with the institution on or before 30 th of April, 2021;

THAT I have submitted credentials or documents pertaining to the qualifications of

Teacher 1 to the Department of Education;

THAT, I also received on January 18, 2021 the appointment to a position of Teacher
1 in Bulacao National High School;

THAT I am requesting for a Clearance to my present employer to be submitted to

DepEd Sorsogon for an added requirements to my appointment in the said
assignment school;

THAT I also offered to be in DATA BASE TECHNOLOGY COLLEGE, INC. every

Saturday to fulfill whatever duty as one of its teacher/Adviser/faculty member;

THAT I will be compensated with seven hundred pesos (700.00) during Saturday
work with working hours of 8:00AM to 5:00PM;

THAT I am executing this affidavit to attest to the truth of the foregoing

circumstances and to attest to the validity and authenticity of the documents
submitted.

IN WITNESS WHEREOF, I have hereunto set my hand this ____day of January
