AFFIDAVIT OF UNDERTAKING FOR UNVACCINATED PERSONNEL

I, PCpl Jen L Lego, of legal age, residing at Brgy. Agsirab Lambunao, Iloilo, a
member of Philippine National Police, presently assigned at Iloilo Police Provincial
Office designated to the position as DRD PNCO, Supply Section after having been duly
sworn in accordance with law do hereby depose and state.

1. That I am one of the unvaccinated personnel of Iloilo PPO;

2. That I failed to avail the vaccine as a part of the government’s effort to fight the
spread of COVID-19 infection in the country;

3. That I am eight (8) months pregnant as of the moment and currently a work from
home;

4. That I believe that the effect of vaccines for pregnant women does not yet provide
safe guaranty and in pursuit of my status considering my own health and specially
safety of the life inside me.

IN TRUTH WHEREOF, I have hereunto sign my name below this 15th day of
